(A) Each application for registration of an agricultural additive shall be accompanied by a registration fee of fifty dollars. Application shall be made on a form supplied by the director of agriculture and shall include:

1. (1) The name and address of the registrant;
2. (2) The additive's brand name;
3. (3) The purpose or proposed use of the additive;
4. (4) Directions for the additive's use;
5. (5) The results or effects that can be expected when the additive is used as directed;
6. (6) Evidence that the additive has been tested for efficacy in the laboratory and in the field, information describing the test methodology used, and a description of the test results;

7. (7) A guaranteed analysis of:

   1. (a) If a chemical product, the name and percentage of each active and inert ingredient;
   2. (b) If a microbiological product, the number and kind of viable microorganisms per milliliter or, if the product is other than a liquid, per gram. The registrant shall notify the director of any change of address within thirty days of the change; failure to notify the director constitutes grounds sufficient for the director to revoke the registration.

(C) The director shall register an agricultural additive unless he determines that:

1. (1) The application for registration is not complete;
2. (2) The additive will not or is not likely to produce the results or effects claimed when used as directed; or
3. (3) The additive is not useful for agricultural production in this state.
