The department of health shall include on its web site information about biological parent's name redaction request forms. All of the following information shall be provided:

1. (A) The purpose of the form;
2. (B) The procedures to be followed and requirements to be met for the department to accept the form;
3. (C) The date when biological parents may begin to file the form with the department;
4. (D) The date when the form may no longer be filed with the department;
5. (E) The procedures to be followed and requirements to be met for having the form removed from an adopted person's adoption file;
6. (F) Any other information the department considers necessary.
