N.Y. Comp. Codes R. & Regs. tit. 9, § 6653.5 – CSE projects in area plans.

(a) An area agency participating in CSE may propose projects to make community services and entitlement programs more accessible to elderly people through improved coordination and delivery of services, to enable them to remain in the community and avoid institutionalization. As necessary to meet project goals and objectives set forth in the area plan, CSE projects may provide new services or expand existing services, and may establish new mechanisms to coordinate existing and new services.
(b) For each CSE project proposed, the area plan shall specify:
1. (1) intended goals and objectives;
2. (2) the elderly population to be served;
3. (3) a timetable not to exceed three years to achieve and evaluate the goals and objectives; and
4. (4) proposed methods to evaluate its effectiveness.
(c) For CSE projects providing new or expanded services, the area plan:
1. (1) must include procedures to assist recipients in obtaining other services and benefits that will assist them in remaining in or returning to their homes; and
2. (2) may include but are not limited to adult day care, homemakers, housekeepers, chore, home health, shopping assistance, counseling, transportation, protective, or other services to maximize an elderly person's independence in the home and community.
(d) CSE projects establishing new mechanisms to coordinate existing and new services:
1. (1) must simplify intake, application or eligibility procedures for services or entitlements, and minimize inconvenience for elderly people seeking and receiving such services and entitlements; and
2. (2) may include but are not limited to case management, centralized intake and screening, centralized information and referral services, focal points and procedures to assist elderly people in finding services, benefits and entitlements.
(e) The office may approve continuation of a CSE project beyond three years if periodic evaluations show that the project effectively improved the delivery of services to the elderly.
(f) In any year for which a county receives EISEP service funding, all EISEP requirements under this Chapter shall apply new CSE clients receiving similar in-home services, noninstitutional respite, and when provided to persons needing and/or receiving in-home services or noninstitutional respite, or case management services.
