(a) Each voucher issued, except special promotional vouchers as approved by the commission, must have printed on its face:

1. (1) the name of the authorized pari-mutuel wagering entity where the voucher was issued;
2. (2) the unique computer-generated voucher number;
3. (3) the date and time the voucher was issued;
4. (4) the number of the ticket-issuing machine; and
5. (5) the dollar amount of the voucher.
