N.Y. Comp. Codes R. & Regs. tit. 9, § 2608.1 – Reapplication submission requirements.

(a) A neighborhood preservation company's contract may be renewed or extended from year to year, based on contractual performance.
(b) The applicant must complete such forms and attachments as prescribed by the division.
(c) Essential components of the reapplication include:
1. (1) recertification of eligibility;
2. (2) quantified, measurable objectives to be accomplished by the completion of the activities proposed in the work plan;
3. (3) proposed budget for the coming contract year; and
4. (4) evidence of the required one-third match;
  (d) Required documents.
  The following documents shall be submitted prior to review of the reapplication:
5. (1) annual certified audit for last completed fiscal year;
6. (2) annual program and fiscal reports for the last completed contract; and
7. (3) quarterly program and financial reports for the second quarter.
