(b) Essential components of the application include:

1. (1) documentation supporting the eligibility of both the applicant and the neighborhood;
2. (2) general goals in effecting neighborhood preservation, stabilization or improvement;
3. (3) quantified, measurable objectives to be accomplished by the completion of the activities proposed in the work plan;
4. (4) budget, specifying the use of funds requested from the division and other financial resources, including the one-third match requirement and staffing pattern; and
5. (5) documentation of community support.
