(b) Permit holders are required to submit a Parking Permit Update form for any of the following reasons:
(9) cancel payroll deduction.
Completed forms must be returned to the parking lot attendant or to the Bureau of Parking Services. Registration and update forms are available from the OGS website, the bureau's central office or the agency parking coordinator. All forms must be submitted through the bureau's central office.
