2. (b) sexual abuse;
3. (c) sexual assaults;
4. (d) employee misconduct;
5. (e) communicable reportable disease;
6. (f) contraband;
7. (g) deaths;
8. (h) escapes;
9. (i) escape attempts;
10. (j) fires;
11. (k) personnel group actions;
12. (l) youth group actions;
13. (m) hostage situations;
14. (n) major disturbances;
15. (o) major maintenance/service disruptions;
16. (p) natural/civil emergencies;
17. (q) attempted suicides;
18. (r) self-inflicted injuries; and
19. (s) serious accidental injuries.

The following incidents shall be reported to the commission pursuant to the requirements of this Part:
