(a) A facility shall develop and implement an admission health appraisal process for each youth which includes, but is not limited to:

1. (1) a health screening which consists of a visual assessment and interview which shall be performed as soon as possible after each youth's admission to a facility, but in no event later than 24 hours after admission;
2. (2) a complete written medical history which shall be obtained for each youth within 72 hours of initial admission to a facility, and shall include assessments of chemical dependency and infectious diseases; and
3. (3) a complete physical examination which shall be provided to each youth within 72 hours of initial admission to a facility, and at yearly intervals thereafter.

(b) Each youth's physical examination shall be conducted by a physician, registered physician's assistant or nurse practitioner, as appropriate, and shall include, but is not limited to:

1. (1) an examination of major body systems;
2. (2) Mantoux tuberculin skin test;
3. (3) sexually-transmitted infections screening;

4. (4) where clinically indicated, screening tests including, but not limited to:

   1. (i) a urinalysis;
   2. (ii) blood chemistry analysis to include lipid screen;
   3. (iii) a chest radiograph;
   4. (iv) a complete blood count with white blood cell differential count;
   5. (v) a pregnancy test for female youth; and
5. (5) a cervical cytology (PAP test) for female youth when, in the medical judgment of the examining physician, registered physician's assistant or nurse practitioner, such a screening is medically necessary and appropriate.
