All papers submitted to the commissioner in connection with an appeal shall be in typewritten form on white paper 8½ by 11 inches in size, and endorsed with the name, post office address and telephone number of the party submitting the same, or, if a party is represented by counsel, with the name, post office address and telephone number of the party' s attorney.
