1. (a) A list of all emission sources at the facility.

2. (b) For each emission source:

   1. (1) the potential to emit of the PAL contaminant;
   2. (2) the applicable requirements for that emission source; and
   3. (3) the baseline actual emissions (with supporting documentation).
3. (c) The methodology that the facility is proposing to use for conversion of the monitoring system data to monthly and annual emissions based on a 12-month rolling total for each month.
4. (d) If the applicant seeks to utilize section 231-9.4(e)(2) of this Subpart as the basis for calculating the reduced PAL level, the applicant shall provide, as applicable, a BACT review in accordance with section 231-8.7 of this Part and/or a LAER analysis in accordance with section 231-6.5 of this Part.
