- (a) a completed renewal application, which will be supplied by the department;
- (b) a completed physician's statement of physical condition, which was signed and dated no more than 30 days prior to submission with the application;
- (c) current American Red Cross standard first aid certificate, or equivalent;
- (d) current American Red Cross CPR certificate or equivalent; and
- (e) the appropriate license renewal fee.
