N.Y. Comp. Codes R. & Regs. tit. 22, § 1851.3 – Pleadings and papersfiled with court.

(a) The court will not acceptsubmissions via facsimile transmission.
(b) Any pleading or paper supplementalthereto, e.g., waiver and consent, renunciation, affidavit etc., thatis dated more than six months prior to the date of filing, is considered staleand shall not be accepted for filing by the court.
(c) No party in interest ina proceeding may act as notary for any papers filed in the proceeding.
(d) Any document submitted forfiling in this court that is not an official Surrogate's Court form,but for which the chief administrative judge has prescribed a form, shallbe on standard 8½″ by 11″ paper, the text must belegible, the standard typeline shall be of 10- to 12-point characters (otherthan instructions), the margins shall be no less than one-half inch, and theform shall have attached thereto the following certification from either theattorney or the party who prepared the document:
“The undersigned hereby certifiesthat the attached document submitted for filing is the same as the prescribedform, and the substantive text of that prescribed form has not been altered.”
