(a) All attorneys, other thanthose whose representation has been noted by a pleading, must file a noticeof appearance and authorization to appear.

(b) Substitutions of attorney.

No subsequent attorney representing a party will be permittedto appear unless there is filed with the clerk a consent to the change signedby the retiring attorney and signed and acknowledged by the party.
