N.Y. Comp. Codes R. & Regs. tit. 22, § 608.1
Any organization which offers prepaid legal services; any nonprofit organization whether incorporated or unincorporated, organized and operating primarily for a purpose other than the provision of legal services and which furnishes legal services as an incidental activity in furtherance of its primary purpose; or any organization which has as its primary purpose the furnishing of legal services to indigent persons, which has its principal office located in the First Judicial Department, shall file with this department a statement describing the nature and purposes of the organization, the composition of its governing body, the type of legal services being made available, and the names and addresses of any attorneys and counselors-at-law employed by the organization or with whom commitments have been made. An updating of this information shall be furnished this department on or before July first of each year and the names and addresses of attorneys and counselors-at-law who rendered legal services during that year shall be included.
