N.Y. Comp. Codes R. & Regs. tit. 18, § 651.2
(a) For purpose of preparation of annual reports for the New York State Legislature, social services districts must report monthly to the office information on families and individuals receiving public assistance benefits, including, but not limited to, the following:
(1) Applicants seeking public assistance including but not limited to:
(2) Recipients of public assistance including but not limited to:
(3) Work activities subsidized or otherwise sponsored by the office or social services districts, including but not limited to:
(4) Unsubsidized work activities including but not limited to:
(5) Recipient training and education activities including but not limited to:
(6) Recipient sanctions including, but not limited to:
(7) Case closings, including but not limited to:
(8) Substance abuse treatment programs including but not limited to:
(9) Pregnancy prevention programs funded through Federal monies received by the State pursuant to title IV-A of the Social Security Act including, but not limited to:
(10) Disabled and work-limited recipients of public assistance pursuant to provisions of chapter 436 of the Laws of 1997 (the Welfare Reform Act) including, but not limited to:
(11) Domestic violence screening process including but not limited to:
(b) The information required by this section shall be collected using the Welfare Management System, the Welfare to Work caseload management system or other automated systems or forms as determined by the commissioner of the office. Social services districts shall assist the office in collecting the information in accordance with instructions from the office.
