(a) Hospitals and sanatoria shall report annually on form H-1 consisting of three sections as follows:

1. (1) general information;
2. (2) service statistics—inpatient; and
3. (3) service statistics—outpatients (excluding dispensary).

(b) Hospitals and sanatoria which are in receipt of public moneys shall also report annually financial data on report form H-2 consisting of three sections as follows:

1. (1) current income;
2. (2) current expenses; and
3. (3) balance sheet as of last day of year.
