(a) current income;
(b) current expense; and
(c) balance sheet as of last day of year.
(d) Each such facility shall also submit an annual report of medical care, services and census data on Form DSS-1113, no later than 45 days following the end of the calendar or fiscal year of operations of such facility (whichever is applicable).

Child-caring institutions and agencies which are in receipt of public moneys shall report on report form CW-3b consisting of three sections as follows:
