(5) requests that the case be closed. If the adolescent fails, without explanation or cause, to keep three consecutive appointments made by the case manager, and advance notice of the appointments was provided to the adolescent and the appointments are documented in the adolescent's individual case record, then the case manager may, but is not mandated to, deem such failure to constitute a request by the adolescent to have his or her case closed. However, the adolescent's failure to complete any portion of his or her personal plan will not be deemed a request by the adolescent to have his or her case closed.
