(a) When an order of support is payable through the SCU, the respondent shall provide the SCU with the following information:

1. (1) name;
2. (2) address of residence;
3. (3) social security number;
4. (4) date of birth;
5. (5) place of employment; and
6. (6) such other data as may be determined by the department to be pertinent.
