(a) Each social services district must, in addition to the requirements set forth in section 313.1 of this Part, provide certain clients with follow-up visits as follows:

1. (1) For persons in living arrangements other than those certified by this office, or the State Office of Children and Family Services, State Office of Mental Health or State Office of Mental Retardation and Developmental Disabilities, social services districts must maintain contact for five years following the discharge or release by:

   1. (i) making quarterly visits to the client in the first year following discharge or release, with the first visit occurring within 30 days of discharge or release; and
   2. (ii) making semiannual visits to the client for the following four years. Social services districts may make more frequent visits and/or continue visits beyond the five-year period if determined appropriate by the district.
2. (2) For persons living in family-type homes certified by the State Office of Children and Family Services, clients must be visited in accordance with the provisions of Part 458 of this Title; provided, however, that such visits must be made at least semi-annually for the duration of the client's placement.
3. (3) Social services districts may visit persons in other office-certified facilities or in facilities certified by the State Department of Health, State Office of Mental Health or State Office of Mental Retardation and Developmental Disabilities if the district determines that such visits are necessary to assure the provision of essential services.

4. (4) For persons in other living arrangements, local districts shall maintain contact for five years following discharge or release by:

   1. (i) making quarterly visits to the client in the first year following discharge or release, with the first visit occurring within 30 days of placement; and
   2. (ii) making semi-annual visits to the client for the following four years. Local districts may visit more frequently and/or continue visits beyond the five-year period if determined appropriate by the district.

(b) Such visits must verify and evaluate:

1. (1) receipt of requested or needed services;
2. (2) adequacy of the present level of care and living arrangements in terms of physical and mental health; and
3. (3) the ongoing appropriateness of the individual service plan.
