N.Y. Comp. Codes R. & Regs. tit. 15, § 1.4
(c) In all other areas of the State, except the City of New York and the Counties of Nassau and Suffolk: A licensee who is employed on a regularly scheduled basis as set forth in subdivision (a) of this section may drive unaccompanied to and from a place of business as set forth in section 1.1(a) of this Part provided such licensee possesses his or her driver's license and either the “Certificate of Employment” (form MV-58A) completed by such licensee's employer, a letter signed by his or her employer containing the date of the letter, the licensee's name, the motorist identification number and job description, the date or days of licensee's employment, the hours of employment, the location or locations of employment and the phone number, as well as the address at which the employer may be contacted by a police officer or magistrate for verification.
