1. (1) The community residence shall maintain a record for each person.
2. (2) The record of a discharged person shall contain a summary of the person's status at the time of discharge, the rationale for discharge, and the discharge plan.
3. (3) Individuals’ personal allowance records shall be maintained in conformance with the regulations of the commissioner.
4. (4) A personnel record for each staff member shall be maintained in accordance with the agency/facility policies/procedures. The personnel records for staff may be kept at a centrally located place in accordance with agency/facility policies/procedures.
5. (5) The community residence shall maintain the necessary statistical records relating to the operation of the facility in accordance with agency/facility policies/procedures.

6. (6) The community residence shall submit information and reports to OPWDD in the format and manner prescribed by OPWDD.

   (b) Standards of certification.

7. (1) The following written information is available for each person in the supervised community residence:

   1. (i) personal identification information which is current, including family information and the responsible party to notify in case of an emergency;
   2. (ii) admission information including the person's medical and developmental history;
   3. (iii) a current developmental disability profile (DDP);
   4. (iv) the recording of illness or injury, including the date and time of occurrence, and action taken, and the appropriate notification made regarding each, during the past year.
8. (2) There are written policies/procedures for the outdated person-specific material not currently being used for program development, delivery of service, or monitoring of a person's status.
9. (3) The personnel records for each staff member shall be maintained by the agency/facility at the location specified in agency/facility policy.
