(d) A clinic treatment program shall offer each of the following services, to be provided consistent with recipients' conditions and needs:

1. (1) health screening and referral;
2. (2) verbal therapy;
3. (3) medication therapy;
4. (4) medication education;
5. (5) symptom management; and
6. (6) psychiatric rehabilitation readiness determination and referral.

(f) A clinic treatment program may also provide the following additional services:

1. (1) case management services;
2. (2) crisis intervention services;
3. (3) clinical support services; and
4. (4) family treatment services.

(j) Admission to a clinic treatment program must occur within the first three visits. A screening and admission note shall be written upon decision to admit which shall include the following:

1. (1) reason for referral;
2. (2) primary clinical and service-related needs and services to meet those needs; and
3. (3) admission diagnosis.

(k) When an admission is not indicated notation shall be made of the following:

1. (1) the reason for no admission;
2. (2) the disposition of the recipient; and
3. (3) any referrals made as appropriate.
