(a) If the amount of the bill awarded under a decision other than a notice of settlement (A-4) is more than $100, the carrier shall promptly:

The parties to disputed podiatry bill proceedings shall each pay to the chair a sum equal to five per centum of the amount payable under such decision or $5, whichever is greater. If there is no amount payable under such decision, or if the disputed bill is determined by notice of settlement (A-4), each party to the proceedings shall pay to the chair a minimum expense fee of $5. The sums so collected shall be transferred to the State Treasury to reimburse it on account of the expense of administration.
