As more fully set forth in sections 324.3, 324.4, 325-1.4 and 442.4 of this Title, insurance carriers, self-insured employers, or third-party administrators must provide the chair or his or her designee in the manner prescribed by the chair with the name and contact information for the point(s) of contact for a prior authorization request (PAR) on or before May 1, 2021.
