A petition for investigation pursuant to section 705 of SERA may be filed with the board by employees, employers or their representatives. The petition shall be in writing. The original shall be signed and verified before any person authorized to administer an oath. The original and four copies of the petition shall be filed with the director. Petition forms will be supplied by the board upon request.
