N.Y. Comp. Codes R. & Regs. tit. 11, § 62-5.2 – Designated fire investigation agency.

(a) Pursuant to section 3415(b) of the Insurance Law, each city with a population of one million or more persons shall designate a local fire investigation agency (the agency).
(b) The agency designated by the Mayor of the City of New York is the Bureau of Fire Investigation of the New York City Fire Department. All required information should be sent to that agency, located at:
