1. (a) power of attorney number;
2. (b) each indemnitor’s name and address;
3. (c) principal’s name;
4. (d) bond amount;
5. (e) amount of premium charged;
6. (f) amount of premium collected;
7. (g) amount of premium remitted to the insurer;
8. (h) amount and description of the collateral required;
9. (i) amount and description of the collateral collected;
10. (j) location of where the collateral is being held;
11. (k) date the principal was released from custody;
12. (l) amount of premium returned;
13. (m) the date of return of premium;
14. (n) date of surrender of principal;
15. (o) disposition of the bail bond;
16. (p) date of exoneration;
17. (q) date of forfeiture;
18. (r) date the collateral is returned to each indemnitor; and
19. (s) description of collateral returned.

Every bail agent shall maintain a bail register that shall be kept on file for a period of at least six years from the date of exoneration or final order of forfeiture and shall include at least the following information, as applicable, for each bail:
