Every commercial insurer providing accident and health insurance in New York State and every article IX-C corporation shall accept the standard claim forms established by the Superintendent of Financial Services when submitted by physicians, dentists or by a hospital for services rendered in New York State to an individual covered by an accident and health policy, or when submitted directly by an insured for covered services.
