(b) If any change has occured in the members of the board of directors, the members or officers of the corporation, the corporation shall file, for evaluation by the department which shall report to the council, information and data with respect to:

1. (1) the character, experience, competency and standing in the community of each such new director or officer;
2. (2) the identity of each new member together with the certificate of incorporation and bylaws of each new corporate member; and
3. (3) such additional information or documentation as may be required
