N.Y. Comp. Codes R. & Regs. tit. 10, § 610.3 – Annual statement.

(a) Each corporation operating a facility shall file annually, upon forms provided by the State Department of Health, a statement identifying the members of the board of directors and the officers of the corporation.
(b) If any change has occurred in the members of the board of directors or officers of the corporation, the corporation shall file, for evaluation by the department which shall report to the council, information and data with reference to:
1. (1) the character, experience, competency and standing in the community of such director or officer on forms provided by the department;
2. (2) such additional pertinent information or documents as requested.
