(a) As indicated on New York State Operating Certificate; the following data is to be reported:

1. (1) certified beds at the beginning of the reporting period;
2. (2) certified beds at the end of the reporting period;
3. (3) date the number of certified beds changed; and
4. (4) certified bed days available during the reporting period.
