(a) Fees.

Upon the filing of an initial application for a license pursuant to this Subpart, the owner shall pay an application fee to the department in the amount of $1,000.

(b) Renewals.

Surrogacy program licensees shall renew their license annually by submitting the information required to be submitted under this Subpart and a renewal fee in the amount of $200. Applications for renewal shall be filed with the department, in the form and manner prescribed by the department, 90 days prior to expiration of the current license.
