1. (a) on or before the 10th day after the end of each month, the information required by the market administrator with respect to producer additions, producer withdrawals, and changes in names of farm operators;

2. (b) on or before the last day of each month such handler's producer payroll for the preceding month, which shall show for each producer:

   1. (1) the total pounds of milk from such producer;
   2. (2) the average butterfat content of such milk: provided, that if not butterfat tests are made on any of the milk received from producers, and if such milk is received by the handler from no more than 10 producers, 3.5 percent shall be reported as the average butterfat test of milk received from producers;
   3. (3) the amount of payment due each producer;
   4. (4) the nature and amount of any deductions and charges made by the handler;
   5. (5) the net amount of payment to such producer; and
   6. (6) Such other information with respect thereto as the market administrator shall require.

Each handler shall report with respect to producers as set forth in subdivisions (a) and (b) of this section:
