Every gas and electric corporation subject to the jurisdiction of the commission shall file with the commission, on a monthly basis, the following information regarding increased fuel costs:

1. (a) what services are affected by the fuel increase;
2. (b) base cost of the fuel;
3. (c) name and address of supplier;
4. (d) point of delivery of all fuel purchased;
5. (e) present average cost of fuel to said corporation;
6. (f) dates used to determine average;
7. (g) amount per unit of consumption affected; and
8. (h) amount of fuel in storage at the end of each monthly period.
9. (i) and such other relevant information as the commission may require.
