N.Y. General Business Law § 899-D
1. An applicant for registration shall submit an application therefor to the secretary of state in such form as shall be prescribed by the secretary of state. An application filed pursuant to this section shall be a public record. An individual who has submitted an application for, and holds a certificate of, registration or licensure as an athlete agent in another state, may submit a copy of the application and certificate in lieu of submitting an application in the form prescribed pursuant to subdivision one of this section.
