Each applicant for a certificate of registration as a registered apprentice must:

1. 1. Be of good moral character and possess temperate habits;
2. 2. Be at least 18 years of age;
3. 3. Fulfill the requirements set forth in this chapter to be a registered apprentice;
4. 4. Present an affidavit from his or her preceptor that such applicant shall enter upon his or her duties as soon as the certificate is granted; and
5. 5. Pay any fees related to the application.
