To be eligible for licensing by the Board as a perfusionist, an applicant must:
- 1. Be a natural person of good moral character;
- 2. Submit a completed application as required by the Board;
- 3. Submit any required fees;
4. Have successfully completed a perfusion education program approved by the Board, which must:
- (a) Have been approved by the Committee on Allied Health Education and Accreditation of the American Medical Association before June 1, 1994; or
- (b) Be a program that has educational standards that are at least as stringent as those established by the Accreditation Committee-Perfusion Education and approved by the Commission on Accreditation of Allied Health Education Programs, or its successor;
- 5. Hold a current, valid certification issued by the American Board of Cardiovascular Perfusion, or its successor organization; and
- 6. Comply with any other requirements set by the Board.
