Upon the request of the juvenile court, a probation officer shall file with the juvenile court a report of:

1. 1. The number of children placed under informal supervision during the previous year;
2. 2. The conditions imposed in each case; and
3. 3. The number of cases that were successfully completed without the filing of a petition.
