Each do-not-resuscitate identification issued by the health authority must include, without limitation:

1. 1. An identification number that is unique to the qualified patient to whom the identification is issued;
2. 2. The name and date of birth of the patient; and
3. 3. The name of the attending physician or attending advanced practice registered nurse of the patient.
