1. 1. Upon furnishing information that his or her identification card is lost or destroyed, the person to whom the original was issued may obtain a duplicate.
2. 2. If the original of a duplicated identification card is subsequently recovered or a lost identification card is found, the person having possession shall return it immediately to the Department.
