1. 1. Every application for an identification card must be made upon a form prescribed by the Department and include, without limitation, a statement from the applicant attesting that the applicant:

   1. (a) Does not hold a valid driver’s license or identification card from any state or jurisdiction.
   2. (b) Is a person experiencing homelessness.
2. 2. When the form is completed, the applicant must sign the form and verify the contents before a person authorized to administer oaths. Officers and employees of the Department may administer those oaths without charge.
