The ballots used for the election held pursuant to NRS 266.029 must:

1. 1. Be in substantially the following form:

   Shall the area described as .........................(describe area) be incorporated as the City of ................. (name of city)?

   Yes ....................

   No .....................

   The voter shall mark the ballot by placing a cross (x) next to the word “yes” or “no.”

2. 2. Contain the statement of the estimated fiscal effect of the proposed incorporation prepared pursuant to NRS 266.0263.
3. 3. Contain a copy of the map or plat that was submitted with the petition pursuant to NRS 266.019 and depicts the existing streets, sewer interceptors and outfalls and their proposed extensions.
