1. 1. If, at any time during his or her appointment, a notary public changes his or her mailing address, county of residence or signature or, if the notary public is a resident of an adjoining state, changes his or her place of business or employment, the notary public shall submit to the Secretary of State a request for an amended certificate of appointment on a form provided by the Secretary of State. The request must:

   1. (a) Include the new information;
   2. (b) Be submitted within 30 days after making that change; and
   3. (c) Be accompanied by a fee of $10.
2. 2. The Secretary of State may suspend the appointment of a notary public who fails to provide to the Secretary of State notice of a change in any of the information specified in subsection 1.

3. 3. If a notary public changes his or her name during his or her appointment and the notary public intends to use his or her new name in the performance of notarial duties, the notary public shall submit to the Secretary of State a request for an amended certificate of appointment on a form provided by the Secretary of State. The request must:

   1. (a) Include the new name and signature and the address of the notary public;
   2. (b) Be submitted within 30 days after making the change; and
   3. (c) Be accompanied by a fee of $10.
4. 4. Upon receipt of a request for an amended certificate of appointment and the appropriate fee, the Secretary of State shall issue an amended certificate of appointment.

5. 5. When the notary public receives the amended certificate of appointment, the notary public shall:

   1. (a) Destroy his or her notary’s stamp and obtain a new notary’s stamp which includes the information on the amended certificate.
   2. (b) Notify the surety company which issued his or her bond of the changes.
