Letters of guardianship shall contain:

1. A. the names, addresses and telephone numbers of the guardian;
2. B. the name, address and telephone number of the incapacitated person; and
3. C. the scope of the guardianship including the specific legal limitations imposed by the court on the powers of the guardian.
