1. A. The primary caregiver's registration agreement is personal, and not transferable to any other person or location.

2. B. A registration will expire automatically at midnight of the expiration date unless earlier suspended or revoked, or:

   1. (1) if the primary caregiver moves; or
   2. (2) changes their name.
3. C. If the primary caregiver moves to a new location or has a change of name, the primary caregiver must register again by submitting a new application and pay the processing charge. The caregiver must report a new location or change of name prior to the occurrence and receive a new on-site health and safety inspection by ECECD.
4. D. A caregiver with only a change in name will not need to pay an amended fee or processing charge.
