1. A. The CTC shall have policies and procedures to stabilize and transfer clients in need of a higher level of care.

2. B. The CTC shall:

   1. (1) discuss recommendations for transfer with the client or client’s legal guardian or agent and upon transfer, notify the client’s legal guardian or agent;
   2. (2) make the determination as to the time and manner of transfer to ensure no further deterioration of the client during the transfer between facilities;
   3. (3) specify the benefits expected from the transfer in the client’s record;
   4. (4) coordinate care with receiving facility prior to transfer; and
   5. (5) send a copy of the client’s record with the client upon transfer.
