Regardless of family income, any parent (guardian) of an eligible student as defined by Subsection B of 7.30.10.7 NMAC may apply for funds for the following expenses:

1. A. cost of a comprehensive eye examination by a participating optometrist or ophthalmologist through referral;
2. B. cost of polycarbonate lenses and frames for eyeglasses or for contact lenses; and
3. C. cost of replacement insurance for lost or broken lenses.
