All non-federal licensed health care facilities in New Mexico shall report to the commission the following data for their prior fiscal year:

1. A. Capital asset disposals and retirements
2. B. Depreciation
3. C. Facility license number
4. D. Facility name
5. E. Fiscal year ending
6. F. Net income (loss)
7. G. Owner(s) of facility
8. H. Total capital asset ending balance
9. I. Total new capital asset purchases
